A cervical pillow is a shape with a job

A cervical pillow is built around one idea: your neck shouldn't hang in the air while your head rests on the pillow. So the pillow has a raised roll where your neck goes and a lower cradle where your head goes. Lie on your back and the roll fills the curve under your neck. Lie on your side and the taller part fills the space between your ear and your shoulder.

That's the whole design. Everything else on a listing — the fill, the cover, the cooling claims, the name — sits on top of that shape.

Two things it is not. It isn't a medical device, and it isn't one standard product. "Cervical" just means the neck. Makers use the word for pillows that look quite different from each other, which is why the table shows the stated shape and loft instead of trusting the name.

Who a cervical pillow suits

A cervical pillow tends to suit people who:

• Sleep mostly on their back, mostly on their side, or switch between the two
• Like a pillow that stays the same height all night
• Don't fold, punch or hug their pillow into shape
• Are willing to try one roll height for a week before judging it

It's a harder fit for stomach sleepers. A raised roll pushes the head up and back when you're face down, and most stomach sleepers want the lowest pillow they can find. If that's you, start with the thin pillows instead.

What the table shows, field by field

The table lists bed pillows whose maker describes them as cervical or contour, or whose stated shape has a center cradle with raised edges. Every field appears only where the maker or the listing states it. A blank means nobody said, not that the answer is no.

Pillow type and shape tell you if it's really cervical

The pillow type column sorts shaped pillows into a few groups: cervical / contour, butterfly, neck roll, arm-hole cutout and shoulder cutout. The shape column is more specific: a center cradle with raised edges, a wave, a butterfly with side wings, a roll.

We fill the shape from the maker's drawing or product name, never from a generic "shape" box on a shopping page. Those boxes sometimes call a cradle pillow "rectangular", which is technically the outline and practically useless.

Loft decides the fit

Loft is how tall the pillow is. On a cervical pillow there are often two numbers: the height of the neck roll and the height of the center. Some makers give both, some give one, some give none. The table shows what's stated, in inches.

Here's the rule. Back sleepers usually want a lower roll. Side sleepers usually want a higher one, and the broader your shoulders, the higher it needs to be. If only one loft figure is listed, it's almost always the tallest point.

Sizes are about your body, not your bed

Most bed pillows come in standard, queen and king, which are about the width of the bed. Cervical pillows often come in body sizes instead, with names like small, medium and large or petite, average and large. The bigger sizes usually mean a taller roll.

Some makers give a measuring method, such as the distance from the base of your neck to the edge of your shoulder. If a maker gives a method, use theirs. If it doesn't, the size name alone won't tell you much, so lean on the stated loft. We don't publish a shoulder-width chart, because the makers don't agree on one.

Fill changes how firm the shape stays

Many cervical pillows are molded memory foam: one solid piece, poured in a mold so the cradle and rolls are part of the foam. Others use latex, shredded foam in a shaped cover, or fiber stitched into channels. The fill column shows what the maker states.

Molded foam holds its shape best. Shredded foam can be adjusted but loses some of the crisp shape. Fiber is softest and the least likely to keep a defined roll. More on the fill itself is on the memory foam pillows page.

Firmness is the maker's own word

Firmness on a pillow is a description, not a measurement. One maker's "medium" can feel like another's "firm", and there's no shared scale. The table shows the maker's word and never ranks by it.

Sleep position is the maker's claim, not ours

The sleep position field shows the positions the maker names for this pillow: side, back, stomach or combination. It's a useful filter. It isn't a promise the pillow suits every body in that position.

Certifications are listed only as the maker states them

Two labels come up a lot on foam pillows:

• CertiPUR-US is a program for flexible polyurethane foam. Accredited labs test foam samples for certain chemicals, heavy metals and emissions, plus baseline physical performance. It covers the foam, not the cover.
• OEKO-TEX STANDARD 100 certifies a textile as tested for harmful substances. On a pillow it usually covers the fabric. A real label carries a certificate number you can check on the OEKO-TEX website.

The table shows either one only when the maker names it for that pillow. Plenty of pillows say nothing, and then the cell is blank.

Two rolls, one cradle and a few variations

Cervical pillows come in a handful of layouts. Here's how to tell them apart:

• Two-roll cradle. A dip in the middle, one roll on each long edge, the rolls at different heights. You choose a height by turning the pillow around.
• Single-roll cradle. One raised neck edge and a sloped back. Simpler, one height.
• Butterfly. A center cradle with two raised wings at the ends, meant to hold the head when you turn to the side. These have their own page: butterfly pillows.

• Adjustable cervical. A shaped cover with removable layers or loose fill, so you can lower or raise the roll.

If you can't decide on a height, the adjustable kind is the honest answer. Check that the listing says the layers or fill are removable. A neck roll pillow is a separate item: a firm cylinder you add to your existing pillow, not a pillow on its own.

Put it on last, and the right way round

One week in 1998 a medical convention filled every room in the hotel, and my floor turned 61 rooms in a single day. I still make a bed corner-first and put the pillows on last. On most beds the pillows are the easy part. A cervical pillow is the exception, because it has a front and a back.

Look for the label or the drawing that shows which edge is the neck roll. The neck edge faces you, toward the foot of the bed. If there are two rolls, start on the one the maker recommends for your position. The how to use a cervical pillow guide walks through it step by step.

Many cervical pillows aren't the standard 20 x 26-inch bed pillow size, and many come with a fitted cover. Check the stated dimensions. A regular pillowcase can bunch around the rolls and hide the shape. Check whether the maker sells replacement covers before you buy a case. (Dev bought a king pillowcase for one. It looked like a sleeping bag.)

Give it a week before you judge it

A shaped pillow feels different on the first night, and that's normal. Your head lands in a dip instead of a flat surface, and the neck edge is firmer than you're used to. Give it about a week before you decide whether the height is right.

If you wake up with your chin pushed toward your chest, the roll may be too tall for how you lie. If your head tips down toward the mattress on your side, it may be too low. Try the other roll first, if there is one. Then check the maker's return or trial terms, which are on the listing, not on our page.

Cervical is a shape, not a treatment

Plenty of people look for a cervical pillow because their neck hurts. Here's what we'll say about that. A pillow can be the right or wrong height for you, and the right height is more comfortable. That's as far as a pillow goes. It isn't a treatment for neck pain, and we don't rank pillows by any relief they claim.

Some listings use health words like "pain relief" or "chiropractor". When the maker does, the table shows those words in quotes, as the maker's own, not as our finding. If neck pain keeps up, wakes you at night or comes with numbness or headaches, that's a conversation for your doctor, not a shopping list. Care: the cover washes, the foam usually doesn't

Read the care tag before you read anything else. Many molded foam cervical pillows have a removable cover that can go in the washer, while the foam itself can only be spot cleaned. Shredded foam and fiber fills vary. A few can go in a washer whole, many can't.

Housekeeping rule: if the listing only says "machine wash", find out whether that means the cover or the whole pillow. Sellers often write it for the cover alone. The care field in the table shows what's stated, and the how to wash pillows guide covers the rest.

Who should skip a cervical pillow

You probably don't need one if:

• You sleep mostly on your stomach
• You move a lot at night and like to bunch the pillow under your head
• You share a pillow habit with a partner who sleeps in a different position
• Your current pillow is comfortable and you only want a fresh one

In that last case, a new pillow of the same height and fill is the simpler buy. Start from the pillows section and pick by sleep position.
