An ergonomic pillow is a shaped bed pillow

Here's the rule. If a bed pillow has a shape built into it — a roll, a dip, wings or a cutout — it belongs on this page. If it's a plain rectangle, it doesn't, however the listing describes it.

The words on the box are loose. Ergonomic pillow, orthopedic pillow, therapeutic pillow and contour pillow all show up on the same kinds of products, often in the same title. None of them is a standard. No agency tests a pillow and grants it the word "orthopedic". So we sort by shape, which you can see in a drawing, and by stated loft, which you can check with a ruler.

What the names on the box usually mean

Shaped pillows collect names the way the archive collects pillowcases. Here's how the common ones tend to be used in listings. None of them is a guarantee of anything, so treat each as a hint and check the drawing.

• Ergonomic. The broadest word. Any shaped pillow, sometimes even a plain one with a firmer edge.
• Orthopedic. Usually a cervical design with a neck roll. Sometimes a knee or wedge pillow, which is why the type column matters.
• Therapeutic. Mostly cervical pillows. The word sounds clinical; it describes the product line, not a result.
• Contour. A smooth wave across the top, usually molded foam.
• Cervical. A center cradle for the head and a raised edge for the neck.
• Orthopedic neck pillow. Either a cervical bed pillow or a travel pillow worn around the neck. Check which.

When a title stacks four of these words together, it's describing one pillow four ways, not four features. Dev counted the words in one listing title once and got to 31 before he found the size. (He has a spreadsheet for that. I didn't ask to see it.)

The fastest way through the noise is the shape column. A drawing doesn't exaggerate. If the listing has no drawing and no stated shape, the pillow doesn't make it onto this page.

Cervical and contour: a roll for the neck

The most common ergonomic pillow. A raised roll runs along the edge that sits under your neck, with a lower area for your head. Cervical designs usually have a distinct center cradle; contour designs are more of a smooth wave. Both are often molded memory foam.

They suit back and side sleepers who want one height all night. See cervical pillows and contour pillows.

Butterfly: wings at the ends

A center dip with raised wings on either side, so the pillow holds your head when you turn from your back to your side. Good for people who switch positions during the night. See butterfly pillows.

Neck roll or bolster: a firm cylinder

A long, round pillow that tucks under the neck, either alone or in front of a regular pillow. People also use one behind the back on a sofa or under the knees. See neck roll pillows.

Arm-hole and shoulder cutouts: room for the bottom arm

A hollow or notch near one end, so a side sleeper's lower arm or shoulder has somewhere to go instead of jamming under the pillow. See pillows with an arm hole and shoulder pillows.

Not in this group

Wedges, knee pillows, leg rests, reading pillows and body pillows are shaped too, but they do a different job. They have their own pages: knee pillows, leg elevation pillows, reading pillows, lumbar support pillows and U-shaped body pillows.

Choose by how you sleep, then by fill

My one opinion on this page: buy a pillow for your sleep position first and your fill second. The loft and the shape decide whether your head lands at the right height. The fill decides how that height feels.

Back sleepers

A back sleeper wants a modest roll that fills the curve under the neck without pushing the chin forward. A cervical or contour pillow on its lower side is the usual fit. Look for back sleeping in the sleep position field and a stated loft.

Side sleepers

A side sleeper needs more height, because the pillow has to fill the space from the ear to the edge of the shoulder. Taller cervical rolls, cutout designs and butterfly pillows with high wings are the shapes to look at. Broad shoulders usually call for the taller option or a larger maker size.

Combination sleepers

If you start on your back and wake up on your side, a butterfly shape or a two-roll cervical pillow gives you two heights in one. An adjustable pillow, where you can take out layers or fill, is another route.

Stomach sleepers

Most shaped pillows aren't built for you. A raised neck roll tips your head back when you lie face down. A thin pillow is usually the better start.

How to read the table

Every field shows only what the maker or listing states. A blank is a blank, not a no.

Pillow type and shape

The type is the group above. The shape adds detail, such as a center cradle with raised edges or a wave. We take it from the maker's drawing or product name. A shopping page's "shape" box sometimes calls a shaped pillow "rectangular", so we don't use it.

Loft

How tall the pillow is, in inches. Shaped pillows can have two heights, one for each roll. When only one number is listed, it's usually the tallest point.

Size

Shaped pillows are often sold in body sizes — small, medium, large, or names like petite and average — instead of standard, queen and king. The bigger size usually means a taller roll. If the maker explains how to measure for a size, use their method. We don't publish a universal chart, because makers don't share one.

Fill and firmness

Fill is what's inside: molded memory foam, shredded foam, latex, fiber. Molded foam keeps a shape best. Firmness is the maker's word, not a measured value, so a "medium" from one maker can feel like a "firm" from another. The table never ranks by it.

Sleep position

The positions the maker names. Useful as a filter. Not a promise for every body.

Health wording

Some listings use medical-sounding words: "pain relief", "chiropractor", "posture". When they do, the health wording column shows the exact phrase in quotes, as the maker's statement. We never treat it as a finding, and we never rank pillows by it.

Certifications

CertiPUR-US covers flexible polyurethane foam: accredited labs test foam samples for certain chemicals, heavy metals and emissions. OEKO-TEX STANDARD 100 covers textiles, usually the cover. The table shows either one only when the maker names it for that pillow.

Orthopedic is a name, not a health promise

People shop for an orthopedic pillow or a therapeutic pillow because something hurts. That's a fair reason to want a better-fitting pillow. But a pillow can only be the right or wrong shape and height for you. It isn't a treatment for neck or back pain, and nothing on this page says otherwise.

Sizes, cases and the bed you already have

A standard bed pillow is 20 x 26 inches, and a standard pillowcase is made for that size, as the US labeling rules' own example shows. Shaped pillows often aren't. Many are narrower or shorter, and many come with a fitted zip cover.

A regular pillowcase over a cervical or butterfly pillow can bunch up and flatten the very shape you paid for. Check the stated dimensions, and check whether the maker sells spare covers. Care is decided by the fill

Read the care tag first. Molded foam usually has a washable cover and a foam core you can only spot clean. Shredded foam and fiber vary by maker. Latex usually can't go in the washer at all.

If a listing says "machine wash" without saying what, assume it means the cover until the maker says otherwise. The care column shows exactly what's stated. The how to wash pillows guide covers the rest.

Give a new shape a week

A shaped pillow will feel odd the first night. Your head goes into a dip and your neck meets a firm edge. Give it about a week before you decide, and try the other roll or orientation before you give up on it. Check the maker's trial or return terms on the listing before you open the box, since some don't take back opened pillows.

Goldilocks tried three beds before one was right. With shaped pillows it's usually the second roll, not the third pillow.

Who should skip an ergonomic pillow

You probably don't need one if:

• You sleep on your stomach most of the night
• You like to fold, scrunch or hug your pillow
• You switch positions often and hate turning a pillow around
• Your current pillow is comfortable and simply worn out
